Office Visit Procedures

1. Bring your insurance cards and picture ID to your appointment.

2. Obtain and bring the prescription from your doctor for any services that are covered by insurance.

3. Make sure your physician documents in your medical chart all of the qualifying conditions that allow you to obtain the durable medical equipment you are seeking. This includes documentation of medical necessity from your physician indicating they believe the durable medical equipment would be beneficial for you based on your diagnosis.

4. Be prepared for your visit. Wear appropriate attire to have a gait analysis done (shorts or loose-fitting pants that can roll up and stay up past your knees) and bring appropriate footwear and socks to your appointment. Please bring extra pairs of shoes that represent what you typically wear.

Returns
Any prefabricated medical devices that are worn are non-returnable due to hygiene.  Custom devices can be adjusted but not returned. Adjustments for custom products must be made within 30 days of dispensing.  Any concerns brought up outside of 30 days may be subject to an office fee, service charge, and shipping fees.

Shoe Return
You have 15 calendar days to return shoes from the date you received them. To be eligible for a return, shoes must be unused and in the same condition that you received it. Your item must be in the original packaging. Your item needs to have a receipt or proof of purchase. Returned items are subject to shipping and restocking charges.

Cancellation & Missed Appointment Policy
Our goal at Sole Remedy is to provide you with convenient, accessible, high-quality medical care. In order for us to assure convenience and accessibility to all of our patients, it is important that patients arrive timely for all scheduled appointments or cancel the appointment 24 hours in advance. This policy allows us to make better use of our available appointments for those patients in need of assistance.

Cancellation of an Appointment
You may cancel your scheduled appointment by calling  Sole Remedy at 508-295-8800 or  855-FOOTRX-9. Follow the instructions on the voice mail if not able to speak directly with a staff member. Appointments are in high demand and your early cancellation will give another individual the opportunity to be seen.

Missed Appointment Policy
 A “missed appointment” is an occurrence where someone does not show up for an appointment and does not cancel the appointment in advance of the scheduled date and time. If you do not show up for your appointment and you do not cancel the appointment 24 hours in advance, you will be subject to an office fee.

Fees for Missed Appointments
Patients will be charged an office fee when they do not present for scheduled appointments or fail to cancel or re-schedule the appointment within 24 hours of the scheduled appointment time.  This fee will not be submitted to your health plan; it will be charged to you the patient.

Cancellation & Missed Appointment Policy
I pre-authorize Sole Remedy to use the payment information (debit card and/or credit card) on file to charge for the applicable fees. If there is no payment information on file, I understand that I will be billed for the applicable fee. I acknowledge that I have read and understood the above policy statement regarding the fees for missed appointments. Additional information is available in the Notice of Privacy Practices which is available in the Sole Remedy office.

Authorization of Billing & Financial Obligation Policy Patient Attestation
I authorize the use of my insurance information to bill my primary and or secondary insurer for services and or supplies rendered by Sole Remedy. I understand a claim will be submitted to my primary and secondary insurer on my behalf and the payment assignment will be accepted by Sole Remedy. I will be financially responsible for any co-payments or deductibles assigned to me by my insurer unless otherwise indicated. I understand that I am responsible for the payment of all products and services not covered by insurance at the time of service. If I am unable to meet my financial obligation, I understand that it is my responsibility to contact Sole Remedy in writing to discuss payment options. I understand that if I fail t meet my financial obligation after 90 days I will be subject to legal action.

It is the patient's responsibility to understand what is covered or not covered insurance covers under their plan. Patients will be responsible for payment of any products or services not covered by their insurance at the time of service.

Payment Policies:
Sole Remedy accepts payment in the form of cash, check, or credit card.* Credit or debit card use is subject to a convenience fee. Payment is due at the time of service.
